The Domino Mi Diabetes Program is an initiative created to improve the diabetic patient population with an uncontrolled condition. This program offers support to diabetic patients with glycated hemoglobin (HGbA1c) levels greater than 9%.

Services:

• Delivery of a glucometer and supply of strips and lancets every 90 days or as recommended by your doctor.

• Monthly educational activities.

• Evaluation and monitoring with the help of a multidisciplinary team.
