Patient Rights and Responsibilities

    • Receive high-quality health care services.
    • Submit complaints or claims to the Health Advocate’s Office via the toll-free Information Line 1-800-981-0031.
    • Receive information from your physician and participate in all decisions related to your medical care and treatment.
    • Choose your Primary Care Physician, Specialist, Laboratory, Pharmacy, and X-Ray provider according to the insurer’s list of health care providers.
    • Be treated in any Emergency Room 24 hours a day, 7 days a week, without needing authorization from your primary physician or insurer.
    • Communicate freely with your physician or other health care provider without fear and in strict confidentiality.
    • Ensure your physician keeps your medical record information confidential.
    • Receive equal treatment and respect from your physician and any other health professional.
    • Not be discriminated against for any reason.
    • Receive services from a specialist on the insurer’s provider list, following the referral procedures of your health plan.

Responsibilities:

  • Inform your Primary Care Physician or specialist about all aspects of your health condition.
  • Notify your physician of any changes in your health condition.
  • Follow the treatment recommended by your primary physician or specialist.
  • Maintain appropriate behavior that does not harm, hinder, or prevent other patients from receiving the medical care they need.
  • Take care of your health and practice healthy lifestyle habits.
  • Report any improper charges or violations of your patient rights to the Health Advocate’s Office.
  • Use medical services, health care facilities, and emergency rooms only when necessary or as required by your health condition.
  • Be familiar with your health plan and the services it offers.
  • Report any fraud related to the services you receive or the health facilities you visit.
  • Respect all health care professionals.

Patient Responsibilities to Inform Your Primary Care Physician:

  • Medications you are taking from other specialists, including natural remedies.
  • Bring consultation reports from external specialists.
  • Hospital discharge summaries and/or Emergency Room visit summaries, including physician recommendations.
  • Information about your self-management.
  • Lab tests and studies (X-rays, ultrasounds, etc.) performed outside the Center.
  • Advance directives.

Information Available for Your Condition, Self-Management, and Preventive Health Care in Spanish and English:

Our center has bilingual staff with excellent communication skills. To request an interpreter, please notify us when scheduling your appointment at our Information and Medical Appointments Department. This service is available for any department or service offered at our Center.

Patient Transition Process from Child to Adult: When the patient turns 17 years old, the transition process begins. The patient and their family are guided to select their primary care physician.

Important: The patient and their family must contact the insurer to make the official change to their health insurance card.
