Sliding Fee Discount Program

The Sliding Fee Discount Program allows patients who do not have health insurance or whose coverage is limited to receive health services at a lower cost. Camuy Health Services, Inc. ensures that patients have access to all primary care services, regardless of their ability to pay. We can offer you a discount on the provision of primary and preventive health services, taking into consideration your family composition and your federal poverty level. The Tiered Discount program is NOT HEALTH INSURANCE, and only provides a discount for services provided by Camuy Health Services, Inc. ,Inc.

Required documents (according to the applicant’s capacity):

• An application must be completed at the Registration Department.
• Evidence of any income you receive (paycheck, social security letter, unemployment letter, etc.)
• Photo ID
• Evidence of medical plan or program appointment with Medicaid.
• Water, electricity or telephone bill that certifies the physical address.

*This list of requirements should not, under any circumstances, represent a barrier for patients

 Who can apply for the discount card?

• Any patient or family unit who wishes and/or requests it.

How can I request the discount card?

• You must go through the Registration Department and complete a socioeconomic evaluation.
• Eligibility determination is based on household size (all immediate family members living in your home) and gross annual income.
• You will need to update your information every year or whenever you have a significant change in income. Your discount will last for 12 months.
